We’re living well but Dying Matters
This film, produced by CHANGE for Dying Matters and the National End of Life Care Programme, is about including people with learning difficulties in discussions around death, dying and bereavement.
It features people with learning disabilities telling their stories and sharing their wishes to support other people with learning disabilities to become more comfortable talking about dying, death and bereavement.
KITE – Keep Improving the Experience
KITE was developed as part of Northumberland’s GP practice accreditation scheme. It involves self-assessment books which practices complete and which serve as a baseline analysis of palliative care delivery.
Continuing Health Care fast-track tool
Accompanies national framework guidance for continuing health care. To be used when a person has primary health needs arising from a rapidly deteriorating condition which may be entering a terminal phase.
Carer’s assessment tool
If someone provides care for a relative or friend, they may be entitled to an assessment of their needs and can request a Carer’s Assessment from the local social services department. Help the Hospices provides a useful outline about the format an assessment may take.

Supportive & Palliative Care Indicators Tool (SPICT)
Developed in Scotland, this clinical tool is based on the premise that clinicians can identify people who are in the next 6-12 months of life based on readily identifiable clinical indicators and use these to initiate a discussion with individuals and families, and screen them for unmet needs.
