Coordination of care
Each person approaching the end of life should receive coordinated care, in accordance with their care plan, across sectors and at all times of day and night.
The Marie Curie Cancer Care Delivering Choice Programme has demonstrated the effectiveness of establishing a central coordinating facility providing a single point of access through which all services can be coordinated.
It is also recommended that locality-wide registers or electronic palliative care co-ordination systems are created for people approaching the end of life, so that they can receive priority care.
To support the implementation of these registers and systems, the National End of Life Care Programme and Department of Health are applying to the Information Standards Board for a new national end of life information standard. This standard will define the core content to be recorded and shared amongst professionals. For further infomation please view the information standard webpages.
