Essex Social Services have developed a team of specially trained community support workers to ensure people with palliative care needs receive coordinated support at the end of their lives.

The team work with the in-house service, independent providers and the community nursing team to make sure all care reflects the cultural and spiritual needs of the individual and their family and that informal carers receive appropriate support.

They also provide practical advice and information on other support services such as bereavement, financial and emotional support and liaise with community nursing teams, especially where there are problems of symptom control.

The scheme, which is funded through Adult Social Care, is currently being rolled out across Essex.

Future plans are to consolidate the service and embed it in a collaborative health and social care approach to providing and commissioning end of life care.
