Cedar Court, a 52-bedded care home (nursing) in Leicester, set up a register of residents to identify, assess and plan for people’s future needs using advance care planning.
The approach helps involve residents and their family in their own care.
It has also improved communication between staff and encouraged greater teamwork.
This has in turn improved the quality of care and co-ordination with the GP practice and led to fewer hospital admissions in the last stages of life.
