Motor Neurone Disease (MND) Supportive and Palliative Pathway Project in Leicestershire and Rutland

Neurology and palliative care services in Leicestershire and Rutland have come together to develop a clear pathway for people with motor neurone disease and their carers from diagnosis to end of life care.

The pathway, which originated from a mapping day involving all interested parties in March 2005, aims to:

  • Promote clear pathways of communication between service providers
  • Promote timely anticipation of evolving need and advanced care planning especially around end of life care
  • Develop systems to avoid unwanted emergency admissions to hospital.

The project, which is staffed by one project worker with secretarial support, has set up a number of multi-professional task and finish groups to focus on some of the issues highlighted at the mapping day.

The work has led to improved collaborative working and communication.

It is currently being assessed on the basis of the Motor Neurone Disease association audit tools and questionnaire and satisfaction surveys to service users and providers.

The multi-professional teams are responsible for implementing the improvements and developments.

It is hoped the work will continue and the services will develop with best practice being benchmarked.

It is also hoped to disseminate some of the work both locally and nationally.
