Delivering Choice - end of life partnership working in Leeds

A range of healthcare organisations in Leeds are now working in partnership to share good practice and improve the care of patients who are dying.

A local end of life steering group now facilitates the partnership, called Delivering Choice, which brings together health professionals from the PCT, the acute hospital, specialist consultants, care homes and Marie Curie.

All the parties involved now work to a shared action plan, which is reviewed on a regular basis and has agreed deadlines.

An integrated care pathway for the dying has been established, ensuring flow charts for medication fit in with Leeds Teaching Hospital Trust and out of hours medical staff, and that all developments and use of documentation are shared across a patient’s journey.

The monthly meetings of the partnership representatives are well attended and have a positive impact on the PCT and wider end of life care agenda.

Care home staff have also been encouraged to fill in a skills gap analysis questionnaire about palliative care education.
