The Care Pathway

The End of Life Care Pathway comprises six steps and was developed to help anyone providing health and social care to people nearing the end of life. The Care Pathway aims to ensure that high quality, person-centred care is provided which is well planned, co-ordinated and monitored while being responsive to the individual’s needs and wishes.

This section of the website contains publications, case studies, tools and other sources of information relevant to each step on the End of Life Care Pathway.

Steps 1 – 6

of the End of Life Care Pathway proceed consecutively. However, assessment and care planning should be revisited as an individual’s circumstances change.
